Documents Required:

• SIM (in case of lost SIM, GD copy is required.)
• Original Ownership / Subscription Paper with 1 recent passport-size photograph
• If the original owner is unable to collect the SIM, an Authorization letter can be given to another person on behalf of owner. Signature on the letter should match the signature on the original subscription.
• Service charge of Tk 1000 (incl. VAT)
• If new SIM needs to be issued, an additional charge of Tk. 68 is required.
